Neck and shoulder pain often overlap. It might begin as a stiff neck after sleep, tension across the shoulders from desk work, pain when turning the head, or a shoulder that has gradually tightened up and started referring pain into the upper back or arm. This is something of a staple in clinic and has a high success rate of resolving with a short course of treatment, even when it has become chronic.

Western medicine usually looks at neck and shoulder pain in terms of muscle tension, joint restriction, posture, overload and, in some cases, nerve irritation. Chinese medicine differentiates more carefully between patterns. One person may have pain driven by tension and stagnation, while another’s symptoms are clearly aggravated by stress, cold, overuse or poor sleep. That matters, because it changes how treatment is given.

In clinic, I often find the best results come from combining acupuncture, massage and moxibustion. Acupuncture can reduce pain and calm an irritated system. Massage helps release the soft tissue tension that so often sits around the neck, shoulders and shoulder blades. Moxibustion then helps eliminate pain which helps to restore movement and reduce the feeling of being stuck. Used together, this trinity can work extremely well for both acute flare-ups and more persistent patterns of pain and stiffness.

Treatment is guided by the nature of the pain and the movement restriction. That may mean tension headaches driven by the neck, tight upper trapezius muscles, pain between the shoulder blades, reduced rotation, or symptoms that started after stress, sport, carrying, sleeping awkwardly or too much time at a laptop.
